# Preparing for a Substance Use Assessment

Published 2026-07-23

A substance use assessment is a structured conversation about alcohol or drug use and its effect on health, responsibilities, relationships, and safety. It may be voluntary or requested by a court, employer, healthcare professional, or another organization.

## Gather the basic information

Confirm the appointment time, cost, payment method, expected length, and whether remote attendance is available. Ask which identification and referral documents are required. If another organization needs the result, bring accurate contact details and any release forms you were given.

## Think through your recent history

It may help to note which substances you have used, how often, in what amount, and when you last used them. Consider any changes in sleep, mood, work, school, finances, driving, health, or relationships. Previous periods of stopping or cutting back are useful to discuss too.

You do not need perfectly precise dates for every event. A clear, honest timeline is more valuable than guessing.

## Prepare questions of your own

- How will the assessment information be used?
- Who will receive the final report?
- What happens if education or treatment is recommended?
- How are scheduling, attendance, and completion documented?

Privacy matters.

Ask how your information is protected and what authorization is needed before records are shared. Some reporting may be required by the organization that requested the assessment.

If you are experiencing withdrawal symptoms or have an immediate medical concern, seek appropriate medical care rather than waiting for a scheduled assessment.
